At Special Topics Cafe on Wednesday, we will talk about the role we play as school nurses in the transition of students from pre-K through 21 years of age. We will hear from the Oregon Center for Children and Youth with Special Health Needs (OCCYSHN) at OHSU about the services they provide. Nurses will have the opportunity to share what helps and hinders the work of transition and brainstorm about solutions as a first step in breaking down the silos of care coordination. Nurses who have stories of successes and challenges during transition (especially as students move into kindergarten and beyond high school) will be welcome to share about these experiences.
